Nobody gave me the answer I needed! Is the FT991A fan temperature controlled? Does it only come on when the rig gets hot? I think the IC7300 fan is always on when on TX. If I was to buy an FT991A it would mostly be used at QRP powers.

I’ve found an on-line review which says: “ A quiet fan WHICH ISN’T ON ALL THE TIME.” (my emphasis)
Regards, Keith G0RQQ
Thanks Keith. It sounds like it comes on when hot.
The Icom fan will get on at full speed when in TX, it doesn't matter if you have pwr at 1% or 100%. (Pwr control is in percent on the Icom). But this doens't answer you question. You need to find someone that has a FT-991A and has experience with it.
You can find a review here: https://pa2old.nl/review-yaesu-ft-991/
In this review something about the fan:
"While transmitting the cooling fan comes on a bit early but it is less offensive than the FT-897 fans in its noise generation."
Well I found a video from a transmitting test as well:

Don't know if the fan really sounds like it does in the video? I hope not...

The fan on teh FT-991 (not a) only comes on when it warms up. If it is on in receive for a while in a warm room, the fan may come on at a low level. Transmitting briefly does not run the fan, but transmitting longer does.
I have never had it run at a level that made enough noise that I cared about it. My old IC-730 fan came on with transmit, and was louder (but still not a real problem).
